Description

Boston Medical Center's (BMC) drop-in series Substance Use Disorder Care Continuum ECHO® (SUD Care Continuum ECHO®) offers training and support in addiction treatment, with a focus on supporting front line addiction treatment workforce. This includes health providers in Acute Treatment Services (ATS), Crisis Stabilization Services (CSS), Transitional Support Services (TSS), Opioid Treatment Programs (OTP), long-term residential program, sober and recovery homes, primary care, and psychiatry. This free, case-based tele-mentoring program is designed to increase the capacity of health care providers to implement high-quality and evidence-based addiction treatment.

Intended audience

Advanced Practice Registered Nurses (APRN), Physicians (MD/DO), Physician Assistants (PA), Nurses (LPN/RN), Social Workers (LCSW/LICSW), Licensed Mental Health Counselors (LMHC), Licensed Alcohol and Drug Counselors (LADC), Certified Alcohol/Drug Counselors (CADC), Community Health Workers (CHW), Recovery Coaches.

Speakers

Andrea is a clinical nurse educator for Boston Medical Center's Grayken Center for Addiction Training and Technical Assistance, where she contributes to the development and delivery of continuing education programs, peer-reviewed publications, evidence-based clinical guidelines, and other resources for providers supporting patients with substance use disorders. She is also a family nurse practitioner at Boston Medical Center, an assistant professor of medicine at the Boston University Chobanian & Avedisian School of Medicine, and conducts research in the use of telehealth and coordinated care for persons with opioid use disorder and depression. Andrea is an active member of the Boston University Medical Group Advanced Practice Provider Council and the Massachusetts Coalition of Nurse Practitioners. Andrea has worked with vulnerable populations for over 10 years including managing substance use disorders and overall healthcare for persons experiencing homelessness with the Boston Health Care for the Homeless Program. She is a term lecturer and preceptor for nurse practitioner students at the Massachusetts General Hospital Institute of Health Professions (IHP) and received her Doctor in Nursing Practice (DNP) degree from the IHP in 2018. She also holds certification as a Certified Addiction Registered Nurse - Advanced Practice (CARN-AP) through the Addictions Nursing Certification Board (ANCB).

Objectives

At the conclusion of this activity, participants will be better able to:

1. Determine which patients may be appropriate candidates for treatment with long-acting injectable buprenorphine (LAIB).
2. Identify the two FDA-approved formulations of long-acting injectable buprenorphine.
3. Develop a basic understanding of the dosing, initiation, and maintenance when using long-acting injectable buprenorphine.

Speakers

Justin is a clinical nurse educator with Boston Medical Center's Grayken Center for Addiction Training and Technical Assistance, where he contributes to peer-reviewed publications, evidence-based clinical guidelines, development and delivery of continuing education programs, and other resources for providers supporting patients with substance use disorders. Justin is also a nurse practitioner at Boston Medical Center and the nursing director of two housing first programs operated by the Justice Resource Institute, Inc. He has expertise in infectious diseases and their co-occurrence with substance use disorders, community outreach, and harm reduction. Justin's work in HIV/AIDS includes being president of the Boston Chapter of the Association of Nurses in AIDS Care, consulting for the New England AIDS Education Training Center, and other positions. He currently sits on the stimulant work group for the Opioid Response Network (ORN), the advisory board of the New England Addiction Technology Transfer Center, and the item writing committee for the Addictions Nursing Certification Board (ANCB). Justin received his Bachelor of Science in Nursing (BSN) from Northeastern University in 2013, his Master of Science in Nursing (MSN) from the University of Massachusetts-Dartmouth in 2020, and his Post-Master's Family Nurse Practitioner certificate from the University of Massachusetts-Boston in 2023. He also holds certification as a Certified Addiction Registered Nurse (CARN) through the Addictions Nursing Certification Board (ANCB), certification as an AIDS Certified Registered Nurse (ACRN) through the HIV/AIDS Nursing Certification Board (HANCB), and is a certified nurse educator (CNE) through the National League of Nursing.

1. Recognize the key principles of contingency management as a treatment for substance use disorder.
2. Name at least two evidence-based protocols for the implementation of contingency management for the treatment of substance use disorder.
3. Examine structural barriers to the implementation of contingency management programs and how to address them for program initiation.

Charmaine is an assistant professor of medicine at the Boston University Chobanian & Avedisian School of Medicine and a recent graduate of an adult nurse practitioner fellowship with the Harvard Medical School Interprofessional Palliative Care Fellowship Program. She spends most of her clinical time at the Dana Farber Cancer Institute on the inpatient palliative care consult team and the outpatient clinic, with additional rotations in community-based palliative care. She also provides transitional, low-barrier substance use disorder care at Faster Paths to Treatment, Boston Medical Center's SUD urgent care center. Charmaine was previously a Clinical Nurse Educator for BMC’s Grayken Center for Training and Technical Assistance, where she contributed to peer-reviewed publications, evidence-based clinical guidelines, development and delivery of continuing education programs, and other resources for providers supporting patients with substance use disorder. She has also worked at the Boston Health Care for the Homeless Program as a respite and primary care provider and has conducted research on the healthcare needs of historically underserved populations.

As part of the twice-monthly SUD Care Continuum ECHO® series, Lena Mittal, MD, FACLP, will review the epidemiology of trauma among parents; how experiences of trauma can impact health outcomes, specifically mental health and SUD related; and what evidence tells us about how we should design and deliver care to improve outcomes for this group. Leena Mittal is the Associate Vice Chair for Diversity Equity and Inclusion and an Instructor in Psychiatry at Harvard Medical School. Additionally she serves as the Program Director for the Women's Mental Health Fellowship at the Brigham and co chairs the Justice Equity Diversity and Inclusion committee in the Department of Psychiatry at BWH. Dr Mittal also serves as the Medical Director for Equity, SUD and Community Partnerships for the Massachusetts Child Psychiatry Access Program for Moms (MCPAP for Moms), an innovative statewide consultation service for providers seeing pregnant and postpartum women with mental health and substance use conditions. Dr Mittal has numerous publications and speaks nationally on the treatment of mental health and substance use conditions throughout pregnancy and the postpartum period as well as racial equity in maternal mental health. Dr Mittal is a leader in Diversity Equity and Inclusion and Women’s Mental Health at the program, state and national level. Dr. Mittal is board certified in psychiatry, consult-liaison psychiatry and addiction medicine.

Mary Wheeler began working in the HIV prevention field in 1999 as a volunteer outreach worker for JRI Health in Boston. From 1999 until 2001, Mary worked as the street outreach coordinator for an outreach program that provided services to sex workers and people who use substances.  From 2001 until 2005, Mary worked for both the Cambridge and Boston syringe services programs prior to working in Lynn.  In 2005, she took a position at Healthy Streets Outreach Program in Lynn, which provides services to sex workers and people who use substances. Healthy Streets is also one of the first MA DPH Naloxone Distribution sites, serving towns on the North Shore of Massachusetts.  Mary is currently the Program Director at Healthy Streets Outreach Program, which is now a program of Health Innovations Inc., where she has been for almost 10 years.

Objectives

Speakers

Brittany (Britt) is a clinical nurse educator for Boston Medical Center's Grayken Center for Addiction Training and Technical Assistance. After starting her professional career at Boston Medical Center, she is thrilled to return to her roots supporting patients with substance use and caring for adolescent patients. She is a doctorally prepared, board-certified family nurse practitioner. Her passion is caring for youth and families, especially those with substance use disorder. Her clinical interests include adolescents/young adults, co-occurring mental health disorders, optimizing safety/overdose prevention, and improving continuity of care among DCF/DYS-involved youth. She has over a decade of experience working in addiction medicine from clinical operations to education/training. She pursued her DNP degree to continue to work to bring best practices into clinical care. Britt has published on various topics, including stigma for youth, caring for adolescents with substance use disorders and the role of addiction training for providers. She has also co-authored a book chapter on caring for adolescents with substance use. She is a member of Sigma Theta Tau International and AMERSA (The Association for Multidisciplinary Education and Research in Substance use and Addiction). She received her Doctor in Nursing Practice (DNP) and Master's in Nursing degrees from the University of Massachusetts, Graduate School of Nursing. She also has a Master's in Medical Science from Boston University Chobanian & Avedisian School of Medicine.

She also practices inpatient medicine for geriatric patients at Boston Medical Center. Dr. Pravodelov leads the Evidence-Based Practice Module for participants within the Center of Excellence in Geriatric Medicine at Boston University Chobanian & Avedisian School of Medicine and is a preceptor for Boston University Chobanian & Avedisian School of Medicine medical students, internal medicine residents, and geriatrics fellows. Dr. Pravodelov serves as a mentor or medical students and residents at Boston University Chobanian & Avedisian School of Medicine and as a coach to internal medicine residents at Boston Medical Center. She is also the faculty advisor of the Bridging Gaps in Care student group at Boston University Chobanian & Avedisian School of Medicine.

Objectives

   At the conclusion of this activity, participants will be better able to:

1. Appreciate the extent of substance use and its effects in the older adult population.
2. Implement screening tools for substance use in older adults in their clinical practice.
