Description

As part of the twice-monthly SUD Care Continuum ECHO® series, Tehya Johnson, MSN, AGPCNP-BC, will describe the history of naloxone use for reversing opioid overdose and outline practical guidelines and recommendations for compassionately responding to opioid overdose. The didactic will be followed by a de-identified patient case presentation from an ECHO® participant and recommendations from the panel of experts and attendees.

The Substance Use Disorder Care Continuum ECHO® (SUD Care Continuum ECHO®) series offers training and support in addiction treatment, with a focus on supporting front line addiction treatment workforce. This includes health providers in Acute Treatment Services (ATS), Crisis Stabilization Services (CSS), Transitional Support Services (TSS), Opioid Treatment Programs (OTP), long-term residential program, sober and recovery homes, primary care, and psychiatry. This free, case-based tele-mentoring program is designed to increase the capacity of health care providers to implement high-quality and evidence-based addiction treatment.

Intended audience

Providers, nurses, social workers, psychologists, peer supports, other clinical staff, and non-clinicians.

Speakers

Tehya is a clinical nurse educator for Boston Medical Center's (BMC) Grayken Center for Addiction Training Technical Assistance, a nurse practitioner board-certified in in adult-gerontology primary care, and clinically practices as a Public Health Nurse Practitioner at BMC's Project Trust. Tehya earned a Bachelor of Science in Behavioral Neuroscience from Northeastern University and a Master of Science in Nursing from the Massachusetts General Hospital Institute of Health Professions.

Objectives

Following this training, participants will have the knowledge to:

Charmaine is an assistant professor of medicine at the Boston University Chobanian & Avedisian School of Medicine and a recent graduate of an adult nurse practitioner fellowship with the Harvard Medical School Interprofessional Palliative Care Fellowship Program. She spends most of her clinical time at the Dana Farber Cancer Institute on the inpatient palliative care consult team and the outpatient clinic, with additional rotations in community-based palliative care. She also provides transitional, low-barrier substance use disorder care at Faster Paths to Treatment, Boston Medical Center's SUD urgent care center. Charmaine was previously a Clinical Nurse Educator for BMC’s Grayken Center for Training and Technical Assistance, where she contributed to peer-reviewed publications, evidence-based clinical guidelines, development and delivery of continuing education programs, and other resources for providers supporting patients with substance use disorder. She has also worked at the Boston Health Care for the Homeless Program as a respite and primary care provider and has conducted research on the healthcare needs of historically underserved populations.

Caroline is Associate Dean for Clinical Faculty Affairs and Clinical Professor at New York University Meyers College of Nursing. She is a scholar, educator, and family nurse practitioner whose career-long passion has been the intersection of health and social justice. For over 15 years, she has focused on the role of nursing in perpetuating and lessening health disparities and has worked to develop innovative teaching, practice, and research interventions to improve student, patient, and community outcomes. Description

As part of the twice-monthly SUD Care Continuum ECHO® series, Sarah Axelrath, MD, FASM, will describe the health and social risks associated with alcohol use and their impacts on individuals and families; review evidence-based strategies to reduce alcohol-related risks across clinical, community and system levels; and discuss practical approaches to support safer alcohol use and recovery goals within addiction care practice. The didactic will be followed by a de-identified patient case presentation from an ECHO® participant and recommendations from the panel of experts and attendees.

Speakers

Sarah Axelrath is a primary care, addiction, and street medicine physician at Colorado Coalition for the Homeless' Stout Street Health Center in Denver, CO. Dr. Axelrath has worked in the fields of homelessness and harm reduction for more than a decade, both in direct service roles as well as in policy advocacy to expand legalized syringe access and naloxone distribution throughout Colorado. After graduating from the University of Colorado School of Medicine through the urban/underserved training track, she completed both her internal medicine residency and addiction medicine fellowship at Massachusetts General Hospital in Boston. There, she also worked with Boston Healthcare for the Homeless to offer mobile, low-threshold harm reduction and addiction treatment services on the streets of Boston. She has since returned to her home state where she serves as an advisory board member for the Colorado Drug Policy Coalition advocating for non-carceral, public health and human rights-informed approaches to substance use. She provides full-time primary and specialty addiction care to people experiencing homelessness and living with substance use disorder by "meeting them where they're at" - in the clinic, at the shelter, or on the streets.

• Summarize WHO drinking risk levels.

Among other things, her work has focused on hospital-wide efforts at Boston Medical Center to reduce readmissions of patients with chronic obstructive pulmonary disease through the creation of educational resources for inhaler teaching and nicotine replacement therapies for patients at the bedside and post-discharge.

Dr. Wurcel is a nationally recognized clinician investigator with over 20 years of expertise in infectious diseases and experience working with people with HIV, people who are incarcerated, and people who use drugs. She joined the faculty of Boston University Chobanian & Avedisian School of Medicine in October 2024 as an Associate Professor and clinician investigator in the Section of General Internal Medicine. Additionally, she provides patient care at Boston Medical Center’s Section of Infectious Diseases. Dr. Wurcel’s research portfolio is characterized by a commitment to health equity, development of systems-based strategies to improve healthcare, and engagement of a broad array of stakeholders. She is Multiple Principal Investigator on three NIH/AHQR R-level grants using implementation science methods and has published over 120 peer-reviewed manuscripts advancing the science of health services research and carceral health care.

Description

As part of the twice-monthly SUD Care Continuum ECHO® series, Andrea Jakubowski, MD, MS, will describe the clinical rationale and evidence for initiating long-acting injectable buprenorphine without prolonged sublingual stabilization in people who inject opioids, and discuss practical strategies for initiating, administering, and following patients started on injectable buprenorphine. The didactic will be followed by a de-identified patient case presentation from an ECHO® participant and recommendations from the panel of experts and attendees.

Speakers

Andrea Jakubowski, MD, MS is a general internist, addiction medicine specialist, and Assistant Professor of Medicine at Montefiore Medical Center/Albert Einstein College of Medicine. Dr. Jakubowski provides primary care and medications for opioid use disorder in a syringe services program and opioid treatment program, and cares for patients on a hospital addiction consult service. Dr. Jakubowski’s research focuses on expanding access to evidence-based treatments for opioid use disorder within and outside traditional healthcare settings. She currently has a NIDA-funded K23 to study implementation of long-acting injectable buprenorphine in primary care and SSP clinics. The C-DIAS Fellowship has provided Dr. Jakubowski with valuable training in the application of IS theories and frameworks and implementation strategy development and tracking, which she is applying in her K23 project. As a part of the C-DIAS Fellowship, she has also had the opportunity to join Dr. Sara Becker’s research group to conduct a study using the IFASIS (Inventory of Factors Affecting Successful Implementation and Sustainment), a novel C-DIAS measure, to examine determinants of implementing a digital contingency management platform in opioid treatment programs.

She is a national leader in understanding and studying substance use disorder and addiction for patients undergoing abortion. Dr. Gerber is division chief of General Internal Medicine. She sees adult patients at Albany Medical Center’s Internal Medicine Group and also serves as vice chair of the Department of Medicine for Faculty Development. As an internal medicine physician, Dr. Gerber diagnoses and treats common medical problems and chronic illnesses in patients aged 18 years and older. She has a special interest in women’s health and in trauma-informed care, which takes trauma and adverse childhood experiences into account as part of a holistic approach to medical care.
