Description

Boston Medical Center's (BMC) 12-part Massachusetts Office Based Addiction Treatment ECHO® (MA OBAT ECHO®) offers training and support in addiction treatment, with a focus on comprehensive care for patients with substance use disorders (SUDs). The free, case-based tele-mentoring program is designed to increase the capacity of primary care teams to integrate addiction treatment using a multidisciplinary care model.

Intended audience

Providers, nurses, social workers, psychologists, peer supports, other clinical staff, and non-clinicians.

Speakers

Judith is a clinical nurse educator for or Boston Medical Center's (BMC) Grayken Center for Addiction Training and Technical Assistance and a board certified Psychiatric-Mental Health Nurse Practitioner who has a private practice and works at an inpatient substance use disorder treatment facility. Judith earned her master’s in nursing from Regis College and is certified as a Certified Addiction Registered Nurse – Advanced Practice (CARN-AP) through the Addictions Nursing Certification Board.

Objectives

At the conclusion of this activity, participants will have the knowledge to:

• Identify common psychiatric disorders that may be present in persons with substance use disorder.
• Initiate and/or provide patient information on commonly prescribed psychiatric medications.

Meg is a clinical nurse educator with Boston Medical Center's (BMC) Grayken Center for Addiction Training and Technical Assistance and a psychiatric mental health nurse practitioner at Boston Medical Center. Prior to obtaining her MSN, she worked as a nurse care manager in the Stimulant Treatment and Recovery Clinic (START) clinic, integrating harm reduction, infectious disease treatment, medical addiction groups, and co-management of stimulant use disorders and mental health conditions. Her clinical expertise includes harm reduction, low barrier treatment, complex care management, early identification and management of acute and persistent stimulant-induced psychosis. She earned both her BSN and MSN from Regis College.

• Recognize episodes of overamping.
• Name two classes of medications effective in treating acute psychotic symptoms of overamping.

Dr. Holt is an Associate Professor of Medicine at Yale School of Medicine, where he also serves as the Associate Program Director for the Primary Care Internal Medicine Residency Program. As an Addiction Medicine specialist, he directs the Addiction Recovery Clinic at Yale and has spoken at numerous regional, national, and international venues on a a variety of Addiction Medicine topics. Dr. Holt has won local and national awards for his clinical teaching, including the New Investigator/Educator Award from the Association for Medical Education and Research in Substance Abuse (AMERSA), the Leonard Tow Humanism in Medicine Award from the Yale School of Medicine, and the Early Career Physician Teaching Award from the American College of Physicians (ACP).

• Recognize the signs and symptoms of alcohol withdrawal syndrome.
• Identify appropriate candidates for outpatient management of alcohol withdrawal syndrome.

Daniel Rodrigues is the Director of Substance Use Disorder and Outreach Programs at Duffy Health Center located in Hyannis. Daniel’s background is as a Licensed Independent Clinical Social Worker, graduating from Smith School for Social Work in 2013. Daniel has worked in the field of substance use disorders and mental health since graduation, having worked in inpatient co-occurring treatment, outpatient residential, and hospital based Social Work on Maternity and Pediatrics, before transitioning to his current role at Duffy Health Center. In his role at Duffy, Daniel provides clinical oversight for all of the health center’s substance use disorder services, and outreach programs, as well as the center's winter weather response program.

• Identify three potential causes of homelessness.
• Recognize considerations specific to providing care to people experiencing homelessness with substance use disorders.
• Describe three strategies for medication management for persons experiencing homelessness with an opioid use disorder.

Joanna D’Afflitti, MD, MPH has served as Medical Director of Respite Programs at Boston Health Care for the Homeless Program since February 2021.  Dually board certified in Internal Medicine and Addiction Medicine, Dr. D’Afflitti received a Master in Public Health from Columbia University in 2001 and a Doctor of Medicine from the State University of New York Downstate College of Medicine in 2007. She completed the General Internal Medicine Residency Program at Brown University/Rhode Island Hospital in 2010. After completing her residency training she served as a Primary Care Physician at the Massachusetts General Hospital Chelsea Healthcare Center from 2010-2014, where she received support from the Kraft Center for Community Health as a Kraft Practitioner to lead efforts to integrate depression and substance use screening and treatment into primary care. In 2014 she accepted the position of Associate Medical Director for Primary Care Quality and Innovation in the section of General Internal Medicine at Boston Medical Center (BMC). She worked at BMC from 2014-2021 as a primary care physician, also serving as Medical Director for the Office-Based Addiction Treatment (OBAT) program from 2018-2021.

• Identify issues where preventative health screening and education may benefit patients in addiction treatment.

Sarah Axelrath is a primary care, addiction, and street medicine physician at Colorado Coalition for the Homeless' Stout Street Health Center in Denver, CO. Dr. Axelrath has worked in the fields of homelessness and harm reduction for more than a decade, both in direct service roles as well as in policy advocacy to expand legalized syringe access and naloxone distribution throughout Colorado. After graduating from the University of Colorado School of Medicine through the urban/underserved training track, she completed both her internal medicine residency and addiction medicine fellowship at Massachusetts General Hospital in Boston. There, she also worked with Boston Healthcare for the Homeless to offer mobile, low-threshold harm reduction and addiction treatment services on the streets of Boston. She has since returned to her home state where she serves as an advisory board member for the Colorado Drug Policy Coalition advocating for non-carceral, public health and human rights-informed approaches to substance use. She provides full-time primary and specialty addiction care to people experiencing homelessness and living with substance use disorder by "meeting them where they're at" - in the clinic, at the shelter, or on the streets.

Objectives

• Describe the health and social risks associated with alcohol use and their impacts on individuals and families.
• Review evidence-based strategies to reduce alcohol-related risks across clinical, community and system levels.

Boston Medical Center's (BMC) 12-part Massachusetts Office Based Addiction Treatment ECHO® (MA OBAT ECHO®) offers training and support in addiction treatment, with a focus on comprehensive care for patients with substance use disorders (SUDs). The free, case-based tele-mentoring program is designed to increase the capacity of primary care teams to integrate addiction treatment using a multidisciplinary care model.

• Differentiate acute, chronic and complex pain presentations and their implications for management in primary care settings.

• Identify the two FDA-approved formulations of long-acting injectable buprenorphine.
